The patient has a carbuncle in the region of posterior edge of mastoid.
Task : Think what serious intracranial complication may develop?
Answer :
- at posterior edge of mastoid process present mastoid foramen where constant mastoid emissary Vn passes
- this Vn connect mastoid venous plexus w sigmoid sinus
- sigmoid sinus directly drain into supr bulb of int jugular Vn which is located at jugular foramen
- also thru jugular foramen, glossopharyngeal Nv ( IX ) and accessory Nv ( XI ) passes
- in carbuncle infection fr mastoid venous plexus → spread to mastoid emissary Vn → thrombosis of sigmoid sinus → involve supr bulb of int jugular Vn → compress int jugular Vn and Nv→ Vernet syndrome appear
- Vernet syndromes ( clinical features ) are :
husky voice – paresis of vocal cord
